Terms & Conditions*
- I am adopting the pet described below through Summit's Sanctuary. The pet is being adopted as a companion and will live as a family member with me inside my home. I will not allow the pet to be used for purposes of vivisection or experimentation.
- I will provide proper and sufficient, food, water, shelter and kind treatment for the pet at all times. I further agree the dog will not be chained at any time as a means of restraint or be a primarily outside dog.
- Summit's Sanctuary has provided me (or will provide me upon arrival of the dog) with the following: Adoption Contract + Health Records
- I agree that a representative of Summit's Sanctuary may make periodic inspections of the pet, my residence and the pet’s living conditions on reasonable notice. I further agree to surrender the pet to Summit's Sanctuary's representative if for any reason he/she considers the conditions provided by me for the pet to be unsatisfactory.
- I will keep a collar and identification tag on the dog at all times and will license the dog with the county/city.
- I agree to immediately notify Summit's Sanctuary if the pet is lost or stolen. I further agree, in the event the animal is lost, that I will make every reasonable effort to attempt to locate and claim the animal, and to immediately retrieve the pet from any public pound or animal shelter when notified the animal is being held at any such location and to pay any costs associated with its recovery.
